The Department of Human Genetics has an exciting opportunity for a Full-time Social Worker to work on the UHealth Campus. The Social Worker provides psychosocial assessments, intervention, treatment, and discharge planning to patients and families to assist them in coping with their hospitalization, illness, diagnosis, or life situations. The incumbent strives to meet the continuing needs of patients post hospitalization.

  • Works with providers from NBS, Long Term Care and General Genetics to assess patients and families regarding need of guidance and interventions

  • Works with agencies to obtain coverage for underinsured and underserved individuals/families in our population for visits, testing, DME and treatment.

  • Coordinates the transition of out-patient to home infusions and vice versa

  • Develops and maintains a list of contacts from Children’s Medical Services and Medicaid to assist patients who would be best served under these plans.

  • Makes and keeps contact with those in our population who are easily lost to practice for various reasons

  • Determines how best to navigate these issues so that the department can provide patient care

MINIMUM QUALIFICATIONS 

  • Bachelor’s degree in Social Work

  • Minimum 1 year of relevant experience

  • Skill in completing assignments accurately and with attention to detail.

  • Ability to analyze, organize and prioritize work under pressure while meeting deadlines.

  • Ability to process and handle confidential information with discretion.

  • Ability to work evenings, nights, and weekends as necessary.

  • Commitment to the University’s core values.

  • Ability to work independently and/or in a collaborative environment.

  • Proficiency in computer software (i.e. Microsoft Office).

Any appropriate combination of relevant education, experience and/or certifications may be considered.
