Employment status: Permanent Full Time (35 hours/week)

Salary band: From $42,048 to $52,560/year

The PATH Program is designed to facilitate a smooth transition from hospital to home and ensures that community support services are in place for clients. Collaborating with the Senior Manager of Operations, the successful candidate will be responsible for the day-to-day coordination of the PATH program in the Sudbury District.

In this role, you will:
Conduct client assessments and ensure excellent client service and maintain positive relationship with other agencies and Red Cross departments.

Develop implementation plans for program delivery;

Follow up on client complaints adhering to the National complaint process;

Complete program administration and provide statistical reports.

Evaluate service delivery and make recommendations for improvement.

Recruit, train, and coordinate Transitional Care Workers and volunteer resources

Maintain alignment with assigned budget;

Doing market research for growth potential of service/initiative.

What we are looking for:
One to three year’s community college diploma in Gerontology or related and between one to three years job related experience or an equivalent combination of education and experience.

Oral and written skills in French and English;

Computer proficiency in word processing, spread sheet, and data base applications (Word, Excel, Power Point, and CIMS);

Supervisory or project management experience, including an ability to lead, motivate, and evaluate;

Ability to develop, implement, and evaluate program plans;

Excellent skills in risk, crisis, and conflict management;

Strong customer service skills;

Current CPR/First Aid certification;

Working conditions:
The work is performed in an office environment that is clean and comfortable. Hours of service are based on 35-hour work weeks, Monday to Friday

A valid driver’s license and access to a reliable required

Ability to work non-traditional hours and travel as required

Please note that there is no relocation package for this position.

This position requires a successful Canadian criminal record, reference check, valid provincial driver’s license, and First Aid certification (training provided).
