The Trainer is a new position and will be a key role in the Alameda County Care Alliance’s expansion to new regions. This person will work closely with the Executive Director and leadership team to identify key churches and other stakeholders, and create a comprehensive training plan that focuses on such topics as advance care planning, understanding hospice and palliative care, and the ACCA’s Advanced Illness Care Program™. The trainer will also coordinate logistics and lead training sessions.
The Alameda County Care Alliance Advanced Illness Care Program™ (ACCA-AICP) is a faith-based, person-centered, lay care navigation intervention serving predominantly African American adults with advanced illness and their caregivers. The ACCA extends capacity for advanced illness care beyond the traditional health delivery system, bridging the gap between health delivery systems, community organizations and faith-based communities. ACCA care navigators provide AICP services to all community members free of charge, regardless of race, ethnicity or religious affiliation. ACCA care navigators address participant needs, provide trusted resources, and empower persons needing advanced illness care and their caregivers in 5 cornerstones: spiritual, health, advance care planning, social, and caregiving. The program is currently in more than 42 churches in Alameda and Contra Costa Counties and is continuing to add new churches, and is expanding into San Francisco and Los Angeles. The program also works in partnership with local health systems to provide our intervention services to their referred patients.
This is a remote position, but will be required to conduct both virtual and in-person trainings. Candidates based in the Alameda or Contra Costa county are preferred. The position is required to conduct in-person trainings in these two counties regularly, and in San Francisco and Southern California occasionally.
This is a Part-Time (50% FTE) position with possibility of increasing FTE if funding permits.
Pay: $65,086 to $79,722 per year. The typical hiring range for this position is $65,086 to $79,722 based on 100% FTE. The starting salary is determined based on the candidate’s knowledge, skills, and experience.
Essential Duties & Responsibilities
Work leadership team to identify key churches and other stakeholders
Create a comprehensive training plan that focuses on such topics as advance care planning, understanding hospice and palliative care, and the ACCA’s Advanced Illness Care Program™.
Outreach and relationship building with pastors and faith leaders in designated communities to build interest in trainings.
Establish and maintain process for calendaring and tracking of all trainings and participants.
Schedule and coordinate all logistics for in-person and virtual training sessions.
Coordinate and participate in delivery of onboarding training of staff in planned new region.
May be asked to assist with coordination and delivery of onboarding training of new staff at ACCA Central.
Develop an in depth understanding of Advance Care Planning and ACCA’s intervention program, which will include completing the Care Navigator onboarding training.
Gain familiarity and comfort with the movie “Extremis” and accompanying facilitator guides developed by the ACCA.
May be asked to participate in bi-monthly Care Team AICP quality improvement trainings.
Deliver presentations and facilitate discussions around end of life topics with diverse audiences with compassion and sensitivity.
Deliver presentations on topics such as Hospice, Palliative Care, Advance Care Planning, ACCA Advanced Illness Care Program and other applicable content areas using existing materials.
Revise/update existing presentation materials, as well as develop new presentations in collaboration with ACCA Leadership team.
Five (5) years of some combination of verifiable experience in the areas of delivering training/presentations, outreach within the faith and health community, and direct engagement with those with advanced stage illness.
Experience with end of life care, individuals with advance stage illness, and/or health care.
Ability and willingness to travel to conduct in-person trainings in the San Francisco Bay Area regularly and in Southern California occasionally.
Bachelor’s degree in human resources, marketing, psychology or other related field.
Excellent presentation skills and comfort with presenting sensitive content.
Ability and comfort to work with pastors and faith leader and clinician community; should enjoy working with faith community.
Strong verbal and written communication and interpersonal skills.
Creative and innovative thinking to keep trainings fresh to hold trainees’ attention.
Proficient in managing virtual platforms such as Zoom and providing technical assistance to attendees remotely.
Proficient computer skills, Microsoft Office Suite (Word, PowerPoint, Outlook, and Excel).
Works well in collaborative, diverse team environment.
Strong written and oral communication skills.
Thrives in a fast-paced, complex, and remote work environment.

This position is classified as exempt based on the job duties. However, based on the FTE, salary level of the employee, or federal/state/local laws, the employee may be classified as nonexempt.
